Sometimes I see "if yes", but I'm not sure if it is ok...

I'd only expect to see/hear "if yes..." or "if no..." when the preceeding sentence specifically askes "yes or no?". Otherwise, I'd expect "if so..." and "if not...".

I'd probably only really expect to see this on a form, rather than hearing it in conversation.
