I'm not sure whether "The photo was made by special effects." is grammatically correct.

I think this sentence should be "The photo was made with special effects. Or if the first sentence is okay it has a different meaning than the second one.

If the first sentence makes sense, can it mean that the photo was entirely generated by the computer, not taken by a person?

The photo was made by special effects sounds like the special effects created the photo. So you're better off using The photo was made using special effects.

The photo was made with special effects sounds fine to me.

Note also the photo was taken with a camera (not by camera)
