I feel it's a combination of the two meanings of yours. This is how I would interpret the sentence:

The narrator and his companion were strangely attracted to the boys' shabby appearance, which Luigi, their driver, strongly disapproved.

Something about the boys: maybe their innocent look, their humble image, drew the narrator and his companion towards them. The driver, however, was disgusted with their looks and was thus warned the narrator not to buy the fruits from them.

I think the use of comma can tell the difference. However I am not sure because even a non restrictive clause can refer to the antecedent which occurs before it.
