The book I'm studying says this is a mixed conditional.

If you were more attentive, you would have passed the exam yesterday.

Is the phrase below also correct?

If you had been more attentive, you would have passed the exam yesterday.

How to know which one to use?

It is a mixed conditional. It's OK.

IF [PAST], WOULD HAVE ...

This is a third conditional. It's OK, too.

IF HAD ..., WOULD HAVE ....

In this particular case, there isn't a particularly good reason that I can see for using a mixed conditional, so I'd use the plain old third conditional.

(I find that a rather unusual example of a mixed conditional to put in a book. There are plenty of others that are easier to make sense of.)
