I was asked to find out wrong sentences but they all looks grammatically correct. Sentence 5 seems a bit awkward though.. Can you tell me whether those 5 sentences are correct?

All those sentences are correct in my opinion (with the small caveat that some people would want a comma in (5), according to traditional rules). If one is supposed to be ungrammatical, it's possible that the setter thinks the use of "since" in (2) is incorrect, though in my opinion "since last month" is acceptable.

This is possible but a little wordy. One use of had would be enough.

In my opinion the past perfect is necessary to clearly convey the meaning that I assume is intended, which is that the accident had already happened at the time that the speaker had to be careful.
