After having reviewed the insurance renewal quote you sent to us for the two vehicles currently on the policy, we've noticed a sharp increase on the premium which is more than 125% compared to last year. We'd like to ask you to explain how one accident last year is penalized with such a heavy penalty?

1. In this case, I think maybe you should just call, unless you aren't fluent enough to speak on the telephone.

3. has v. has been - both are okay. Present perfect is not wrong.

As a general rule, never write if you don't have to.
