The given (Do not use "given" or "provided" or anything else except the number of charts, if there is more than one.) bar chart illustrates (Illustrate is a verb for pictures, diagrams, maps and other pictorial figures. Do not use it for math plots.) the Survey (Survey is not a proper noun. It is incorrect to capitalize it in the middle of a sentence.) results of Percentage of resident's Purchasing and habits of consuming Coffee and tea in five important Australian States. (There are no states plotted. These are cities.)

It is evident that going to a cafe is the most common trend (There is no trend here. You can see trends only when there are many points of time. ) having Coffee or tea in last 4 weeks, except in Adelaide. Additionally, the Percentages of Purchasing instant Coffee always higher (ungrammatical - missing verb.) rather than buying others.
Sydney and Melbourne, it’s worth noticing that, (Do not write these little side notes. Just describe the chart and the data on it. Do not make deductions or conclusions.) while the ratio ( A ratio is expressed as two numbers such as 10000:1, 20:1 or 9:2 or 4:5. A ratio is not a fraction, proportion or percentage. ) of Sydney's individual (not good English. This is one person in Sydney.) drinking Coffee Or tea almost 6I% (That makes no sense) , Whereas, the Melbourne (wrong phrase) Slightly higher 64%. In Sydney and Melbourne, " in contrary, (I do not understand this.) Collecting (Wrong word. Also, wrong capitalization) instant Coffee above 45%, but, using fresh coffee in those cities almost 43% and 42% respectively.
(ungrammatical - no verb)

But, moving to the other cities Having coffee or tea at a cafe accounted for the largest Proportion of the Citizen ( wrong capitalization) in Hobart (There is more than one citizen in Hobart.) which, followed by the city of Brisbane almost 55%, but, Adelaide people’s taking (wrong expression) coffee or tea under 50%. At the same time, buying instant coffee or tea almost 53% (ungrammatical - missing verb.) at Hobart and Brisbame which compared to 50% nearly at Adelaide. However, a collecting fresh coffee in Brisbane and Adelaide around 34%, roughly increased 38% at Hobart.

Learn the rules of capitalization.

1. Every sentence begins with a capital letter.

2. Names of places, people, days of the week, months, and titles of books are capitalized. These are "proper nouns". Read this resource: https://chompchomp.com/terms/propernoun.htm

Do not capitalize these words, except if they are the first word of a sentence: survey, purchase, coffee, tea, state, city, and percentage.

Many of your sentences are poor English. You omit required verbs.
