We shall go at once. Is the question tag shall we?

Yes.

I don't hear 'shall' a lot in N. America.

It is really not relevant in contemporary everyday English. People hardly use "shall" anymore, and the formulaic question tag seems even more bizarre, something out of a Victorian-era novel.
