The stroke preceded the health deterioration.

It seems to me that Past Perfect should be used in the after-clause.

When there are more than two events to contend with, it may depend on which event the writer takes as the anchor event. In your interpretation the anchor event is the deterioration of health. In the writer's interpretation it's the death.

I'd say both versions are possible.
