I am preparing a methodology and questionionare for a survey. Would somebody please help me with the English?

1. In this survey new medicine is defined as a medicine that will available for doctors to prescribe or already available for doctors to prescribe for a period of not more 24 months. Old medicine is referred to a medicine that already available for doctors to prescribe for a period of more than 24 months.

2. What is your overall comment regarding the discussion?

3. On a scale of 0 to 10, please rate the quality of the discussion? (10 is high and 0 is low, please choose one). Please indicate your assessment by circling one of the points on the scale below.

4. Doctors will be needed to fill in the questionnaire following each sales visit focusing only on the main product/claim discussed in the discussion.

In 1. I would put "new medicine" and "old medicine" in quotation marks to indicate their differences. In 2 it would probably be better to ask for comments in the plural. In 3, remove the question mark which is unnecessary. In 4. Do you mean to say "Doctors will need" or that the person conducting the survey needs to have doctors to fill in the questionnaires; put a comma after "sales visit" to clarify the statement.

In 4, I am trying to say that the person conducting the survey needs to have doctors to fill in the questionnaires. Does my original sentence have different meaning?

Thanks for your comments. I really aprreciate that. I did not put the sentences in order. I fully in agreement with your statement that the question on the overall comment should be in the last.

For the sentence number three, I have changed to :

Please rate the quality of the discussion. Please indicate your assessment by ticking the appriate box.

The discussion was : A) Poor B) Good C) Excellence D) Outstanding

Yes it does. Normally the sales representatives discuss at least one product in each visit.

In this survey “new medicine” is defined as a medicine that will be marketed soon or has been released recently or never heard before.“Old medicine” is referred to a medicine that that has been released for sometime or has come to the knowledge before.
