When we say 'medium', we rarely add the word 'size'.

A- I usually wear a medium size,
B- I take a medium size.
C- I'm a medium size.
D- I'm medium size.
E- I'm size medium.
F- My size is medium.

Is this for a class? if you are not being formal, most of those are okay.

A- tells the person what you usually wear, but also shows that you are unsure whether that is the correct size right now
B- tells the person that in general you buy mediums. Once again, this doesn't mean you are sure you want a medium this time. You could use "I will take a medium size" to show that you are going to take that size this specific time.
C- tells the person the size you are and implies that you can only wear medium so the person will probably give you a medium
D- This and E are the only incorrect ones, but if you change it to "I'm medium sized" this works. same meaning as C.
E- Is not correct. This implies that size medium is your name, or that you are the definition of "size medium"
F- same meaning as C

other possibilities: "I wear a medium"
