If you wish, but 'she is tall and slim' is a native choice.

It is awkward, but you may, because 'as well as' is a subordinator, making 'I' the sole subject of the verb.
It strikes me as one of those things that old-fashioned teachers use as a test item. I can't imagine many people actually saying or writing it.
