Truth is a noun:
My mother told me to always tell the truth.
The truth is that some birds can fly and other birds cannot.

True is an adjective:

She dreams of her one true love.
It is true that some birds cannot fly.
Shakespeare's play, Romeo and Juliet, is based on a true story.

Yes, that is the difference. Adjectives describe nouns:

a good idea (good = adjective, idea = noun)
a big black fat dog (big, black, fat = adjective, dog = noun)
