How should I understand "as received"? How should I understand the "as"?

It means "in the state they were in upon their arrival here". They are trying to say that whatever information is on the records they receive, that's what they record whether it is right or wrong and whether or not it even makes any sense. You can argue with Stella on the phone later until you're blue in the face, but it won't get you anywhere. You have to submit documentary proof if there was something wrong and you want to correct it.
As for "as", I didn't know what they meant until I read the rest of it. They were too terse. They need to be specific. Anyway, the usual online dictionaries didn't have this definition. A safari deep into the OED yields "With the subordinate clause, esp. a participial one, reduced to the complement: in the way or condition that it is (was, has been, etc.)."
