We often find captions under photographs such as "A 24 year old Marlon Brando on the set of ....".
Why do we use the article 'a' ? Is it the short for " A photograph of 24 year old Marlon Brando ....." ?

It could be, but it is also sometimes used when the writer/speaker is thinking of another person at one of various ages or stages of life.
