At age 5 my niece was taken to a speech therapist for a check-up on the recommendation of her teacher.

Checked up sounds odd to me. If you delete up, it sounds much better.

Two alternatives.

At age five, a speech therapist checked my niece on the recommendation of her teacher.

Because of her teacher's recommendation, my five-year-old niece was checked (for language problems) by a speech therapist.


You can use check up in a different setting.

Because she hadn't heard from him in two hours, she went into his bedroom to check up on him.


Generally speaking, you should write out single-digit numbers (0-9).
