i feel like want to cry making my report..i'm confusing of using those words..
which one is correct?

1.The pedestrian link design was changed due to column is/was located in the road median
2.The pedestrian link has been changed due to column is/was located in the road median

1. The beam was constructed with size 600mm
2. The beam has been constructed with size 600mm
or can I just use
3. The beam constructed with size 600mm

when to use is changed, was changed and has been changed?

is changed -- Use this for always: Any design is changed whenever an error is discovered

was changed -- Use this to state the simple past fact: The design was changed yesterday

has been changed -- Use this when you wish to stress the recentness of the past fact, or or to stress that the task is complete.

It sounds that was changed/was __+ed and has been changed/has been __+ed have similar meanings..isn't it?-- Often, yes. The event is the same, but the present perfect connects the event to Now in some way.

and 1 of my friend told me that 'has been __+ed' is something that always happen. for example 'has been constructed' means from past until today, the construction still not finish..is it true?-- Not necessarily true. That is only one use of the present perfect, as in 'I have eaten three bowls of rice so far'-- the eating may continue. If I say, 'I have eaten three bowls of rice and I am stuffed', then the eating is finished.
I think that you should to use 'was changed' on the past tense and 'has been changed' on the progressive present .

Please note that "has been changed" is the present perfect (passive), not a progressive tense. Progressive tenses have an "ing" form. It is being changed.

The beam was constructed with ... Here you are calling attention to events in the past. These things happened. The speaker will think back in time and imagine these things happening.

The beam has been constructedHere you are calling attention to the present-day results of work done in the past. The speaker will think of how these things look now and imagine how they appear with these changes.

Both are correct. It just depends what you want the reader to think about: workmen doing their jobs to change the design and construct the beam? OR how the link design and the beam look now? If you're telling a story of all the things that were done, you need the first formulation. If you're describing what these things are like now, you need the second.

The difference between "was changed" and "has been changed" is two-fold.

1) "Has been changed" indeed refers to a recent action, as it was explained above;
2) Past Simple/Indefeinite includes both aspects, i.e., you can't understand if a process is meant or a completed action. Compare:
The table is made of wood - The key is kept under the rug.
Hence, you might want to use "has been changed" in order to be understood exclusively in Perfect Aspect, something the Past Simple does not allow you to.
