Is the first sentence clear enough to advise a customer not to use a current selection form:

Please note that the selections to be made will change with the changes that you are wanting to make, and we will provide a revised selection form.

or

Please note that the selections to be made will changes with the changes that you are wanting to make, and we will provide a revised selection form to replace the current one.

I'm trying to explain to a customer that they should disregard a selection form that had been given to them at one time and that it will be replaced by a new one because they are making changes to a project.

Why not keep it simple?

eg You are using the wrong form,. We will send you a new one.
