I have called 1-800 ... number and they have filed the form for me and now it is in your computer system. I was reffered to your departament so you can start investigation on my case.

I am enclosing a copy of my last statement and copy of my tax return.

I'm not sure what a earning development work sheet is, so I'm not sure if this is entirely correct.

I have called the telephone number provided on the worksheet, and they filed ?? a corrected form?? ?? the form to correct the earnings ?? for me and told me that it is now in your computer system. But I'm a little confused - "they" completed a form and this form is now in this new department's computer system. Do you want them to do something with this form to get you a revised worksheet? Or do you want them to investigate? What do you want them to DO?

I want them to investigate on my case. To do so they need "the form" that was completeted during my phone conversation. That's what I've been told. Thank you again for your help.
